Form 5862

If you are having difficulty producing a document to prove your Missouri Residency, you can use this form to certify someone’s residency with the support of a Missouri organization. For more information on how to use this form, click here.

Birth Certificate

Application

In Missouri, you can either mail your notarized request in or walk into any local public health office to have your Birth Certificate printed. Each county has their own application but many use the state application. Search our map on the “Get Help” tab to find applications for each county.

Out of State

Many people will need help getting their birth certificate from another state. Use this guide to quickly find the state you need, their unique application link, cost, timing, and notes from programs that might help you be successful.

Social Security Number

The Social Security Card Flowchart is a guide for people to navigate what documents they need in order to get their ID. This can be used directly by a guest or as a reference for a service provider.

Application

Use this application if you are going to the Social Security Administration in person. You may also apply for your SSC online. You need an ID to have your SSC reissued. There are some additional options if you do not have an ID. Talk to your local SSA office for advice.

Payment

Youth BC

Homeless or unaccompanied youth are eligible for a free Birth Certificate. They need to submit this form along with their Birth Certificate application.

Free ID to Vote

Because of legislation that went into effect in August of 2022, an ID is required to vote. Missouri will help people with one ID in their lifetime at no cost for the purpose of voting. Find more information at the Secretary of State’s website.
